Travel Registration Information/Instructions - NJYS

Required Procedures Detailed Below for All Teams

NJ Youth Soccer, NJYS, has updated the required documentation for players and teams to register to play in a Travel soccer league for the Fall 2010 season. Some forms have been updated to reflect changes for registering teams online.

Please follow the below detailed instructions. Your team’s PM will be in contact to collect the required documentation.

For ALL Travel Players:

1. Download form #1, the NJ Youth Soccer Player Membership form. Only complete the personal information. Do NOT complete the League, League #, Club or Club #. Print and sign your name. Print your child’s name, have your child sign. Date the form for both signatures.

2. All players will need a 1" photo of the player.

3. All players that played on teams that played in the Morris County Youth Soccer League, MCYSL, must download and complete the MNJYSA SAGE form, #3. This includes the following teams: SPF Lightning, SPF Blue Pirates, SPF Blue Tigers, SPF Cougars.

For NEW Travel Players:

1. Download and complete the US Youth Soccer Medical Release form. This form must be notarized.

2. Download the MNJYSA SAGE form. One parent must print and sign their name. A parent should print the players name. That player must sign the form.

3. All NEW players will need a copy of their birth certificate.

Travel Player Registration and Payments

For Your Ease and Convenience

Registration for the Travel Program must be done online. Follow the Instructions for registering and paying online:

1. Select the link below and proceed through the normal on-line registration.

2. Log-in to the registration site.

3. Select the child that is to be registered.

4. U9 thru U14 players select the Travel Program.

5. U8 players select the Travel-Lite Program.

6. U15 and above select the Spring Travel Program.

7. Continue the registration process and make the payment.

Before you can select to pay the deposit amount, you must first select the program and the full amount. After you proceed, you will be given an option to pay just the deposit amount. If you have any questions, please email Tom DiGiacomo.
